Overview

delamanid + optimized background regimen is a combination therapy used for the treatment of multidrug-resistant tuberculosis (MDR-TB). Delamanid (Deltyba), developed by Otsuka Pharmaceutical, is a bicyclic nitroimidazooxazole small molecule that acts as a prodrug, activated by the bacterial enzyme deazaflavin-dependent nitroreductase (Ddn). Once activated, it inhibits the synthesis of methoxy-mycolic and keto-mycolic acids, which are essential components of the Mycobacterium tuberculosis cell wall. The optimized background regimen (OBR) refers to a personalized combination of other anti-tuberculosis drugs, such as fluoroquinolones and second-line injectables, selected based on the patient's specific drug resistance profile and World Health Organization (WHO) guidelines. This combination is approved for use in both adult and pediatric patients with pulmonary MDR-TB when an effective treatment regimen cannot otherwise be composed for reasons of resistance or tolerability.
