This is a combination immunotherapy and chemotherapy regimen consisting of a personalized dendritic cell-based precision T cell therapy targeting tumor-specific neoantigens, combined with the chemotherapeutic agent gemcitabine. The dendritic cell (DC) component involves isolating patient-derived DCs, loading them ex vivo with tumor-specific neoantigens (mutated peptides unique to the patient's cancer), and re-infusing them to stimulate an antigen-specific immune response. These DCs present the neoantigens via MHC class I and II molecules to activate both CD8+ cytotoxic T cells and CD4+ helper T cells, promoting a targeted anti-tumor immune response[1][3][4]. Gemcitabine is a nucleoside analog used as standard chemotherapy in various cancers; it inhibits DNA synthesis leading to apoptosis of rapidly dividing tumor cells. The rationale for combining these modalities is that gemcitabine can reduce immunosuppressive elements in the tumor microenvironment, potentially enhancing the efficacy of immunotherapy by facilitating better infiltration and activity of activated T cells.
