Overview

Denosumab + cholecalciferol is a combination therapy consisting of a RANK ligand (RANKL) inhibitor and a vitamin D3 supplement. Denosumab is a human monoclonal antibody that binds with high affinity and specificity to RANKL, a protein essential for the formation, function, and survival of osteoclasts. By inhibiting RANKL, denosumab reduces bone resorption and increases bone mineral density. Cholecalciferol (Vitamin D3) is co-administered to maintain calcium homeostasis and mitigate the risk of severe hypocalcemia, which is a significant potential side effect of denosumab, particularly in patients with impaired renal function. This specific combination regimen is being investigated by The First Affiliated Hospital of Nanjing Medical University for the treatment of osteoporosis in patients with uremia and secondary hyperparathyroidism, where traditional bisphosphonates may be contraindicated or less effective.
