denosumab + eldecalcitol is a combination therapy being investigated for the treatment of osteoporosis, particularly in patients with type 2 diabetes who are at high risk for fractures. Denosumab is a fully human monoclonal antibody that binds to the receptor activator of nuclear factor kappa-B ligand (RANKL), a protein essential for the formation, function, and survival of osteoclasts; by inhibiting RANKL, denosumab reduces bone resorption and increases bone mass. Eldecalcitol is an active vitamin D3 analog that enhances bone mineral density and reduces bone turnover by binding to the vitamin D receptor (VDR). This combination is being studied by Peking Union Medical College Hospital to evaluate its efficacy in improving bone density and reducing fracture risk in the diabetic population, which often presents with distinct bone quality challenges.
