This drug is a combination therapy consisting of the synthetic glucocorticoid dexamethasone and the local anesthetics bupivacaine and lidocaine. It is currently being evaluated by the University Tunis El Manar as a perineural adjunct to an axillary plexus block for patients undergoing hand surgery. The primary objective of adding dexamethasone to the standard anesthetic mixture (bupivacaine and lidocaine) is to prolong the duration of the nerve block and specifically to prevent or mitigate "rebound pain," which is the intense pain that often occurs as the effects of regional anesthesia wear off. Dexamethasone acts as a potent anti-inflammatory agent by activating glucocorticoid receptors, while bupivacaine and lidocaine provide the primary anesthetic effect by blocking voltage-gated sodium channels to inhibit nerve signal transmission.
