Overview

This is a combination drug consisting of dexamethasone, domperidone, and granisetron. It is used primarily for the prevention and management of nausea and vomiting, particularly in settings such as chemotherapy-induced nausea and vomiting (CINV), postoperative nausea and vomiting (PONV), or other situations where multiple antiemetic mechanisms are beneficial. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory properties. Its antiemetic mechanism is not fully understood but may involve inhibition of prostaglandin synthesis, reduction of serotonin release in the gut, or direct action on the central nervous system to suppress emesis[4]. - **Domperidone** is a dopamine D2 receptor antagonist that acts peripherally (mainly at the chemoreceptor trigger zone outside the blood-brain barrier) to block dopamine-mediated stimulation of the vomiting center[4]. - **Granisetron** is a selective 5-hydroxytryptamine 3 (5-HT3) receptor antagonist that blocks serotonin receptors both centrally in the chemoreceptor trigger zone and peripherally in the gastrointestinal tract, thereby inhibiting visceral afferent stimulation of emesis[3][4]. The combination leverages different mechanisms to provide broad-spectrum control over nausea and vomiting. Clinical studies have shown that combinations including granisetron plus dexamethasone are more effective than single agents for preventing emesis after surgery or during chemotherapy[1][2][5]. Domperidone may be added as rescue therapy when additional control over symptoms is needed[2].
