Dexamethasone + exercise is a combination therapeutic intervention being developed by the M.D. Anderson Cancer Center for the treatment of cancer-related fatigue (CRF) in patients with advanced cancer. Dexamethasone is a potent synthetic glucocorticoid that acts as an anti-inflammatory agent by binding to and activating the glucocorticoid receptor. In this specific clinical context, a short course of oral dexamethasone (4-8 mg twice daily) is paired with a standardized four-week physical activity program consisting of aerobic and resistance exercises. The rationale behind this combination is that dexamethasone provides a 'priming effect' by reducing systemic inflammation and improving immediate energy levels, which facilitates better adherence to and efficacy of the exercise program. Clinical trials, including Phase II/III studies, have demonstrated that this dual approach significantly improves fatigue scores and physical function compared to exercise or dexamethasone alone.
