Overview

Dexamethasone + intravenous immune globulin is a combination therapy consisting of the synthetic glucocorticoid dexamethasone and pooled human immunoglobulins administered intravenously (IVIG). This regimen is used for its synergistic anti-inflammatory and immunomodulatory effects. Dexamethasone acts by suppressing multiple inflammatory pathways through glucocorticoid receptor activation, reducing cytokine production, capillary permeability, and leukocyte migration. Intravenous immune globulin provides passive immunity by supplying exogenous IgG antibodies that modulate the immune response via Fc receptor blockade, neutralization of autoantibodies or pathogens, inhibition of complement activation, and modulation of B-cell function. The combination has been studied in several autoimmune or inflammatory conditions including Kawasaki disease[1], pediatric autoimmune encephalitis[3], opsoclonus-myoclonus syndrome[2], relapsed immune thrombocytopenia (ITP)[6], and acute transverse myelitis[7]. Clinical studies suggest that this regimen can accelerate resolution of systemic inflammation in Kawasaki disease[1]; improve neurological outcomes in pediatric autoimmune encephalitis with favorable safety profile[3]; reduce neuroinflammation markers in OMS when combined with rituximab as triple therapy[2]; and provide more rapid platelet recovery than monotherapy for relapsed ITP without increased adverse events[6].
