This is an investigational combination therapy involving the intra-arterial delivery of dexamethasone and ketorolac specifically targeting the sphenopalatine ganglion (SPG). Developed by researchers at Weill Medical College of Cornell University, the treatment is being evaluated for patients with refractory chronic migraine, cluster headache, and trigeminal neuralgia. Dexamethasone, a potent synthetic glucocorticoid, and ketorolac, a nonsteroidal anti-inflammatory drug (NSAID), are infused directly into the internal maxillary artery to modulate the SPG, a key neural structure in the trigeminal-autonomic reflex. This localized delivery aims to provide potent anti-inflammatory effects while minimizing systemic exposure in patients who have failed standard-of-care treatments.
