Dexamethasone + nalbuphine is an investigational combination regimen consisting of the synthetic corticosteroid dexamethasone and the mixed opioid agonist-antagonist nalbuphine. In clinical studies sponsored by Mansoura University, this combination is administered via different neuraxial routes—epidural dexamethasone and intrathecal nalbuphine—to provide synergistic analgesia for patients undergoing lower abdominal oncologic surgery. Dexamethasone functions as a glucocorticoid receptor agonist, exerting anti-inflammatory and analgesic effects, while nalbuphine acts as a kappa opioid receptor agonist and a mu opioid receptor antagonist, specifically targeting visceral pain. This dual-modality approach aims to prolong the duration of effective analgesia, lower postoperative pain scores, and reduce common opioid-related side effects such as nausea and vomiting.
