dexamethasone + neomycin sulfate + polymyxin b is a fixed-dose ophthalmic combination product composed of a corticosteroid and two antibiotics, marketed notably as **Maxitrol** and also available as generic ophthalmic suspension and ointment products. Dexamethasone suppresses ocular inflammation through glucocorticoid receptor signaling, while neomycin inhibits bacterial protein synthesis by binding the bacterial ribosome and polymyxin b disrupts the bacterial cell membrane, providing complementary anti-infective coverage. The product is used topically in the eye for steroid-responsive inflammatory ocular conditions when bacterial infection is present or there is a risk of bacterial ocular infection, including inflammatory conjunctival and corneal conditions, chronic anterior uveitis, and ocular injury associated with chemical, radiation, or thermal burns or foreign body penetration. It is active against common ocular pathogens including *Staphylococcus aureus*, *Escherichia coli*, *Haemophilus influenzae*, *Klebsiella/Enterobacter* species, *Neisseria* species, and *Pseudomonas aeruginosa*, but does not provide adequate coverage against *Serratia marcescens* or streptococci including *Streptococcus pneumoniae*.
