Overview

Dexamethasone + tranexamic acid is a combination of two pharmaceutical agents frequently used perioperatively to improve recovery outcomes after major surgeries such as total hip or knee arthroplasty and other procedures associated with significant inflammation and bleeding. Dexamethasone is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressive properties, acting primarily by inhibiting phospholipase A2 activity and downstream inflammatory mediators. Tranexamic acid is an antifibrinolytic agent that inhibits the activation of plasminogen to plasmin, thereby reducing fibrinolysis and blood loss. The combination has been shown in randomized controlled trials to reduce postoperative inflammatory markers (such as C-reactive protein and interleukin-6), pain, postoperative nausea and vomiting (PONV), fatigue, analgesic/antiemetic requirements, without increasing the risk of complications compared to either agent alone[1][2][5][7][9]. This regimen is considered an effective strategy for enhanced recovery after surgery.
