The combination of dexmedetomidine, esketamine, and lidocaine is an intravenous anesthetic and analgesic regimen being investigated for its opioid-sparing effects in patients with obesity undergoing bariatric surgery. Developed and studied by Yan'an Hospital of Kunming City, this multimodal approach leverages the distinct mechanisms of its three components to optimize perioperative recovery. Dexmedetomidine, a selective alpha-2 adrenergic agonist, provides sedation and analgesia without significant respiratory depression. Esketamine, the S-enantiomer of ketamine, acts as a potent NMDA receptor antagonist to prevent central sensitization and hyperalgesia. Lidocaine, a voltage-gated sodium channel blocker, contributes systemic anti-inflammatory and analgesic properties when administered intravenously. The primary goal of this triple-drug combination is to reduce postoperative pain and opioid consumption, thereby accelerating recovery and minimizing opioid-related complications in the bariatric surgical population.
