Overview

dexmedetomidine + mannitol is a combination therapy being evaluated by Cairo University for the attenuation of increased intracranial pressure (ICP). Dexmedetomidine is a potent and highly selective alpha-2 adrenergic receptor agonist that provides sedation and analgesia; in the context of neuro-anesthesia, it induces cerebral vasoconstriction and decreases cerebral blood flow, which helps in lowering ICP. Mannitol is an osmotic diuretic that creates an osmotic gradient across the blood-brain barrier, drawing water out of the brain parenchyma into the intravascular space, thereby reducing cerebral edema and ICP. This combination is specifically being studied in patients undergoing robot-assisted laparoscopic prostatectomy (RALP) to counteract the ICP-elevating effects of steep Trendelenburg positioning and carbon dioxide pneumoperitoneum.
