This multimodal analgesic combination consists of dexmedetomidine, ropivacaine, and ketorolac, developed by Massachusetts General Hospital for the management of postoperative pain. It is administered via a rectus sheath block to provide localized pain relief and reduce the consumption of systemic opioids following surgery. Ropivacaine acts as a long-acting local anesthetic by blocking sodium channels, ketorolac provides anti-inflammatory and analgesic effects through the inhibition of cyclooxygenase enzymes, and dexmedetomidine, an alpha-2 adrenergic agonist, serves as an adjuvant to prolong the duration of the block and enhance the sedative/analgesic effect.
