Overview

Dexmedetomidine + ulinastatin is a combination of two pharmaceutical agents used primarily in perioperative and critical care settings. Dexmedetomidine is a highly selective alpha-2 adrenergic receptor agonist that provides sedative, anxiolytic, and analgesic effects without significant respiratory depression. Ulinastatin is a serine protease inhibitor with anti-inflammatory properties, commonly used to reduce systemic inflammation and organ injury during major surgeries or critical illness. The combination has been studied for its ability to reduce inflammatory responses, oxidative stress, and postoperative complications such as cognitive dysfunction or myocardial injury in patients undergoing procedures like one-lung ventilation, heart valve replacement surgery, acute aortic dissection management, and colorectal cancer surgery[1][2][3][4][5]. Mechanistically, dexmedetomidine acts centrally on alpha-2 adrenergic receptors to inhibit sympathetic activity while ulinastatin inhibits various proteases (e.g., trypsin) involved in inflammation.
