Dextran 40 + dextran 1 is a combination therapeutic regimen used primarily for plasma volume expansion and the improvement of microcirculation while mitigating the risk of severe hypersensitivity reactions. Dextran 40 is a low molecular weight branched polysaccharide that increases plasma oncotic pressure, drawing interstitial fluid into the intravascular space and reducing blood viscosity by inhibiting erythrocyte aggregation and interfering with fibrin clot structure. Dextran 1 is a very low molecular weight (1 kDa) dextran fraction that functions as a monovalent hapten inhibitor. It is administered as a bolus injection shortly before the infusion of Dextran 40 to bind and neutralize pre-existing anti-dextran antibodies, thereby preventing the formation of large, cytotoxic immune complexes that trigger dextran-induced anaphylactoid reactions (DIAR). This sequential administration is a standard clinical protocol to ensure the safety of dextran-based volume replacement therapy.
