Dextran 70 + glucose (commonly referred to as Dextran 70 in 5% Dextrose) is a sterile, non-pyrogenic intravenous solution used as a plasma volume expander. It consists of dextran 70, a high-molecular-weight branched polysaccharide (average molecular weight of 70,000 Daltons) produced by the fermentation of sucrose by *Leuconostoc mesenteroides*, and glucose (dextrose). The formulation acts as a colloid, increasing the oncotic pressure of the blood and pulling water from the extravascular space into the blood vessels. This expansion of plasma volume improves hemodynamics, cardiac output, and tissue perfusion in patients suffering from hypovolemic shock due to hemorrhage, surgery, or trauma. While it is included on the WHO List of Essential Medicines, its clinical use has declined in favor of other crystalloids and colloids due to risks of anaphylaxis, renal dysfunction, and interference with blood cross-matching and coagulation.
