Overview

Dextrose + levobupivacaine is a combination formulation used primarily for spinal (intrathecal) anesthesia. Levobupivacaine is the S-enantiomer of bupivacaine, an amide-type local anesthetic that blocks voltage-gated sodium channels in neuronal membranes, inhibiting nerve impulse conduction and producing loss of sensation and motor function in the area supplied by the affected nerves. Dextrose is added to make the solution hyperbaric, which allows for more predictable spread within the cerebrospinal fluid based on patient positioning. This combination is used to achieve rapid onset and prolonged duration of sensory block with reduced motor blockade compared to racemic bupivacaine or other local anesthetics. The addition of dextrose does not alter the mechanism of action but modifies physical properties (density/baricity) for clinical effect[3][5].
