Overview

Diatrizoate meglumine + diatrizoate sodium is a water-soluble, ionic, high-osmolarity iodinated contrast medium (HOCM) primarily used for radiographic imaging of the gastrointestinal tract and as an adjunct in computed tomography. Beyond its diagnostic utility, it is frequently employed for its therapeutic osmotic properties. With an osmolarity of approximately 1900 mOsm/L (roughly six times that of extracellular fluid), it draws significant volumes of water into the intestinal lumen. This osmotic effect helps to dilute bowel contents, reduce bowel wall edema, and stimulate peristalsis, making it effective in the non-surgical management of adhesive small bowel obstruction (ASBO), postoperative ileus, fecal impaction, and meconium ileus. It is typically administered orally or rectally and is preferred over barium sulfate when bowel perforation is suspected due to its water solubility and safety profile in the peritoneal cavity.
