Diazoxide + metformin is a combination of two small molecule drugs with opposing effects on blood glucose regulation. Diazoxide is a potassium channel activator that inhibits insulin release from pancreatic beta cells, leading to increased blood glucose levels. It is primarily used to treat hypoglycemia due to hyperinsulinism and can also act as a direct-acting vasodilator for hypertensive emergencies[2][5]. Metformin is a biguanide antihyperglycemic agent widely used as first-line therapy for type 2 diabetes mellitus; it lowers blood glucose by decreasing hepatic gluconeogenesis, reducing intestinal absorption of glucose, and increasing insulin sensitivity in peripheral tissues[4][6]. The combination may be considered in specific clinical scenarios such as research settings or rare cases where both hyperinsulinemic hypoglycemia (requiring diazoxide) and type 2 diabetes (requiring metformin) coexist. However, diazoxide can antagonize the effect of metformin by raising blood sugar levels, so their concurrent use requires careful monitoring[1][8].
