Overview

Diclometh is an investigational topical gel formulation combining the non-steroidal anti-inflammatory drug (NSAID) diclofenac and the opioid receptor agonist methadone. Developed by Mech-Sense at Aalborg University Hospital, it is designed to provide local analgesia for neuropathic pain while minimizing systemic side effects. Diclofenac acts by inhibiting cyclooxygenase enzymes (COX-1 and COX-2), thereby reducing prostaglandin synthesis, while methadone acts as an agonist at mu-opioid receptors and an antagonist at NMDA receptors. The combination has been evaluated in Phase 2 clinical trials using human experimental pain models (capsaicin- and NGF-induced pain) to assess its efficacy, safety, and systemic absorption.
