Overview

Dihydroartemisinin-piperaquine + trimethoprim/sulfamethoxazole is a combination chemoprevention regimen evaluated for the prevention of malaria in HIV-infected pregnant women and their infants. The regimen combines dihydroartemisinin-piperaquine (DHA-PQ), an artemisinin-based combination therapy (ACT), with daily trimethoprim-sulfamethoxazole (TMP-SMX), which is the standard-of-care prophylaxis for HIV-infected individuals. Dihydroartemisinin generates reactive oxygen species that damage parasite proteins and membranes, while piperaquine interferes with heme detoxification. Trimethoprim and sulfamethoxazole act synergistically to inhibit folate synthesis by targeting dihydrofolate reductase and dihydropteroate synthase, respectively. This specific combination was studied in the Phase 3 PROMOTE Birth Cohort 2 trial (NCT02282293) sponsored by the University of California, San Francisco (UCSF) in Uganda.
