**dihydroergotamine + propyphenazone + caffeine** is a combination pharmaceutical drug formulated to treat acute migraine headaches and possibly other types of pain. Dihydroergotamine is an ergot alkaloid that constricts blood vessels by acting as a partial agonist and antagonist primarily at serotonergic and adrenergic receptors, thereby alleviating migraine. Propyphenazone is a non-steroidal anti-inflammatory analgesic and antipyretic, which inhibits prostaglandin synthesis to reduce pain and inflammation. Caffeine is a methylxanthine stimulant that acts as a vasoconstrictor and amplifies the analgesic effect of propyphenazone; it also counteracts sedation from analgesics. This combination is designed to provide enhanced pain relief for severe headache and migraine, particularly when monotherapies are not sufficient. The inclusion of all three is supported by evidence that caffeine amplifies both the antinociceptive and the vascular effects of the other components[14][13]. This exact triplet is rare; most commonly, dihydroergotamine is combined with caffeine, or propyphenazone is combined with caffeine and paracetamol in products like Saridon. The triplet has been noted in literature as a formulation for rapid-onset, multi-mechanism migraine control[13]. Indications primarily include migraine and vascular headaches, though propyphenazone combinations are also used for other pain conditions.
