Diltiazem + lidocaine is a combination drug typically formulated as a topical ointment or gel for rectal use. It combines two active pharmaceutical ingredients: - **Diltiazem** is a calcium channel blocker that relaxes smooth muscle, particularly the anal sphincter, reducing anal canal pressure and promoting increased blood flow to facilitate healing of conditions such as anal fissures and hemorrhoids. - **Lidocaine** is a local anesthetic that provides pain relief by numbing the affected area. This combination is primarily used to treat chronic anal fissures, external thrombosed hemorrhoids, pruritus ani (itching), proctalgia fugax (episodic rectal pain), and other related anorectal discomforts. Diltiazem addresses the underlying spasm and promotes healing by relaxing muscles and improving perfusion; lidocaine offers symptomatic relief from pain through local anesthesia. The combination may also be used in certain procedural settings (e.g., transrectal ultrasound-guided prostate biopsy) to improve patient comfort[2][3][5][6][7].
