Overview

The combination of diltiazem hydrochloride and nitroglycerin is a drug combination used primarily for cardiovascular conditions, particularly angina. This combination leverages the complementary mechanisms of both medications to provide enhanced therapeutic effects. ## Description Diltiazem hydrochloride and nitroglycerin is a combination therapy that produces greater reduction in myocardial oxygen demand than either drug alone. Diltiazem is a calcium channel blocker that works by inhibiting calcium influx into cardiac and vascular smooth muscle during depolarization, resulting in vasodilation and reduced blood pressure. Nitroglycerin is a nitrate that dilates veins and arteries, reducing preload and afterload on the heart. When used together, these medications maintain coronary blood flow without increasing determinants of myocardial oxygen demand. Clinical studies have shown that this combination reduces systolic blood pressure by approximately 27% without significant changes in heart rate, decreases left ventricular end-diastolic pressure by about 36%, and reduces the rate-pressure product by 22%. The combination maintains coronary flow and cardiac output while providing greater anti-anginal effects than monotherapy with either agent. ## Mechanism of Action The combination works through dual mechanisms: 1. **Diltiazem** inhibits calcium movement across cardiac and vascular smooth muscle, causing vasodilation and reducing blood pressure. It also has effects on AV nodal conduction and cardiac contractility. 2. **Nitroglycerin** primarily dilates veins, reducing preload, and to a lesser extent dilates arteries, reducing afterload. This decreases myocardial oxygen demand and increases oxygen supply to the heart. ## Clinical Use This combination is primarily used for: - Management of chronic stable angina - Treatment of variant (Prinzmetal's) angina - Patients who remain symptomatic despite monotherapy ## Side Effects The combination may cause: - Hypotension (potentially more pronounced than with either drug alone) - Dizziness - Feeling faint - Headache - Constipation (primarily from diltiazem) - Flushing ## Precautions Caution is advised as this combination can cause significant blood pressure reduction. Patients should be monitored for excessive hypotension, especially when therapy is initiated.
