Overview

dinoprostone + Foley catheter is a combination of a pharmacological agent and a mechanical device used for cervical ripening and labor induction in pregnant women with an unfavorable cervix. Dinoprostone, a synthetic analogue of the naturally occurring prostaglandin E2 (PGE2), acts as an agonist at prostaglandin E receptors (EP1, EP2, EP3, and EP4). Its primary action in this context is to stimulate the degradation of collagen in the cervix, leading to softening and effacement (ripening), while also promoting uterine smooth muscle contractions. The Foley catheter is a mechanical device inserted into the endocervical canal; inflation of the balloon provides direct mechanical pressure on the internal os and lower uterine segment, which stimulates the endogenous release of prostaglandins and oxytocin. This specific combination was the subject of a randomized clinical trial at the Zeynep Kamil Maternity and Pediatric Research and Training Hospital, which demonstrated that the dual approach significantly shortens the time from induction to delivery compared to using the PGE2 vaginal insert alone.
