Disitamab vedotin + penpulimab is an investigational combination therapy consisting of two agents: - **Disitamab vedotin** is a humanized anti-HER2 antibody-drug conjugate. It targets human epidermal growth factor receptor 2 (HER2) and delivers the cytotoxic payload monomethyl auristatin E (MMAE) via a cleavable peptide linker. The mechanism involves HER2-mediated internalization of the ADC, release of MMAE inside tumor cells leading to microtubule disruption and cell death, as well as bystander effect and immunogenic cell death. - **Penpulimab** is a fully human IgG1 anti-PD-1 monoclonal antibody immune checkpoint inhibitor that blocks PD-1 on T cells to enhance antitumor immune responses. The combination leverages complementary mechanisms—direct cytotoxicity from disitamab vedotin and immune activation from penpulimab—to treat cancers with HER2 expression or low HER2 levels. This regimen has been studied in neoadjuvant settings for cisplatin-intolerant bladder urothelial carcinoma and in breast cancer with low HER2 expression[1][3][6]. Early clinical data show high objective response rates and manageable safety profiles.
