Overview

Disodium cromoglycate + theophylline is a fixed-dose combination of two small molecule drugs used primarily in the management of asthma and other allergic airway diseases. Disodium cromoglycate (also known as sodium cromoglycate or cromolyn sodium) acts as a mast cell stabilizer, preventing degranulation and release of inflammatory mediators such as histamine. Theophylline is a methylxanthine bronchodilator that works by inhibiting phosphodiesterase enzymes (primarily PDE3 for bronchodilation and PDE4 for anti-inflammatory effects), increasing intracellular cAMP, relaxing bronchial smooth muscle, and exerting additional anti-inflammatory actions through histone deacetylase-2 activation. The combination has been shown to be more effective than either agent alone in improving symptom-free days and peak expiratory flow rates in patients with chronic asthma[1][4][5]. Both agents are typically used when inhaled corticosteroids or β2-agonists are insufficient to control symptoms.
