**DKN-01 + folinic acid + fluorouracil + irinotecan + oxaliplatin + bevacizumab** is an investigational combination therapy designed for advanced gastroesophageal adenocarcinomas and potentially other gastrointestinal cancers. DKN-01 is a humanized IgG4 monoclonal antibody that binds and neutralizes Dickkopf-related protein 1 (DKK1), a Wnt/PI3K pathway modulator associated with poor prognosis and immunosuppressive tumor microenvironment. The combination includes folinic acid (leucovorin), fluorouracil (5-FU), irinotecan, oxaliplatin—standard components of FOLFIRINOX or FOLFOX/FOLFIRI chemotherapy—and bevacizumab, a vascular endothelial growth factor inhibitor. DKN-01 enhances PD-1 blockade efficacy by promoting anti-tumor immune infiltration and may benefit patients with high tumor DKK1 expression.
