DKN-01 + pembrolizumab is an investigational combination therapy for cancer. **DKN-01** is a fully humanized IgG4 monoclonal antibody that neutralizes dickkopf-related protein 1 (DKK1), an extracellular protein that modulates Wnt/β-catenin signaling implicated in tumor immunity and progression. **Pembrolizumab** is a monoclonal antibody immune checkpoint inhibitor targeting programmed death 1 (PD-1) receptor to enable anti-tumor T-cell responses. The combination is being evaluated in advanced cancers, notably gastroesophageal (GEJ/GC) and esophagogastric cancer (EGC), to enhance immunotherapy benefit, especially in patients with high tumoral DKK1 expression. DKN-01 is administered intravenously (common doses 150–300 mg on Days 1 and 15 of a 21-day cycle), with pembrolizumab 200 mg intravenously on Day 1. This regimen was well-tolerated in trials, with treatment-related adverse events including fatigue, anemia, asymptomatic liver function changes, and gastrointestinal symptoms. No synergistic immune toxicity or infusion reactions were reported[1][3].
