This multimodal treatment regimen is designed for the management of high-risk localized or locally advanced prostate cancer. It integrates docetaxel, a taxane-based chemotherapy that stabilizes microtubules to prevent cell division, with androgen suppression therapy (AST) and external beam radiation therapy (RT). AST, typically involving LHRH agonists, serves to deplete testosterone levels and inhibit androgen receptor signaling, which is a primary driver of prostate cancer growth. The addition of docetaxel to the standard AST and RT backbone was evaluated in the Dana-Farber Cancer Institute trial 05-043 and the Phase III NRG Oncology RTOG 0521 trial. Results from these studies indicated that the triplet combination significantly improves overall survival and reduces the incidence of distant metastasis compared to the standard doublet of hormone and radiation therapy alone.
