This neoadjuvant chemohormonal therapy (NCHT) regimen, studied at RenJi Hospital, is a combination treatment for locally advanced, high-risk prostate cancer. It consists of the taxane chemotherapy agent docetaxel administered intravenously, paired with a comprehensive androgen deprivation therapy (ADT) blockade using the oral androgen receptor antagonist bicalutamide and the subcutaneous GnRH receptor agonist goserelin. Prednisone is also included to mitigate docetaxel-induced toxicity and suppress adrenal androgen production. The regimen is typically administered for 4 to 6 cycles prior to radical prostatectomy. Clinical trial results from RenJi Hospital demonstrated that this intensive neoadjuvant approach significantly improves 3-year biochemical recurrence-free survival compared to standard neoadjuvant hormonal therapy alone by targeting both rapidly dividing cells and the androgen signaling axis that drives prostate malignancy.
