Overview

DoCaGem is a combination chemotherapy regimen consisting of docetaxel, carboplatin, and gemcitabine, developed for the treatment of high-risk epithelial tumors of Mullerian origin. This regimen was evaluated in a Phase I dose-escalation study led by the Beth Israel Deaconess Medical Center for patients with newly diagnosed ovarian epithelial cancer, fallopian tube cancer, primary peritoneal carcinoma, and endometrial cancer. Docetaxel, a taxane, acts by stabilizing microtubules and inhibiting their disassembly, which leads to cell cycle arrest and apoptosis. Carboplatin is a platinum-based antineoplastic agent that produces inter-strand and intra-strand DNA cross-links, thereby inhibiting DNA synthesis and transcription. Gemcitabine is a pyrimidine nucleoside analog that is incorporated into DNA, causing chain termination, and also inhibits ribonucleotide reductase, reducing the pool of deoxynucleotides available for DNA repair and synthesis. The combination aims to leverage these distinct mechanisms to enhance anti-tumor activity in aggressive Mullerian tumors.
