The combination of docetaxel, carboplatin, inetetamab, and pyrotinib is an intensive chemo-immunotherapy regimen being investigated for the neoadjuvant treatment of HER2-positive breast cancer. This four-drug combination, often referred to as the TCIP regimen in clinical trials, leverages the cytotoxic effects of docetaxel (a microtubule stabilizer) and carboplatin (a DNA alkylating agent) alongside dual HER2-targeted blockade. Inetetamab is a novel recombinant humanized anti-HER2 monoclonal antibody with an optimized Fc region to enhance antibody-dependent cell-mediated cytotoxicity (ADCC), while pyrotinib is an irreversible pan-ErbB receptor tyrosine kinase inhibitor (TKI) that targets HER1, HER2, and HER4. By combining a monoclonal antibody with a small-molecule TKI and doublet chemotherapy, the regimen aims to achieve high rates of total pathological complete response (tpCR) in patients with early or locally advanced HER2-positive breast cancer.
