Overview

The mDCF regimen is a modified chemotherapy protocol developed by Memorial Sloan Kettering Cancer Center (MSKCC) for the treatment of gastric and gastroesophageal junction (GEJ) adenocarcinomas. It consists of docetaxel, cisplatin, and 5-fluorouracil (5-FU), often administered alongside leucovorin. This regimen was specifically designed to mitigate the high levels of toxicity, particularly grade 3/4 neutropenia and gastrointestinal side effects, associated with the standard DCF regimen. Docetaxel, a taxane, functions by stabilizing microtubules and inhibiting cell division. Cisplatin is a platinum-based alkylating agent that induces DNA cross-linking, thereby triggering apoptosis. 5-Fluorouracil is an antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis. The regimen has been evaluated in Phase II clinical trials (e.g., NCT00515411), including cohorts where it is combined with trastuzumab for patients with HER2-positive metastatic disease.
