This quadruple combination regimen is being investigated as a neoadjuvant treatment for high-risk prostate cancer. It consists of the microtubule-stabilizing taxane chemotherapy **docetaxel**, the potent androgen receptor antagonist **darolutamide**, the small-molecule translation elongation inhibitor **homoharringtonine** (also known as omacetaxine mepesuccinate), and **androgen deprivation therapy** (ADT), specifically using LHRH analogs like goserelin. The rationale for this intensified regimen is to overcome the efficacy limitations of standard neoadjuvant ADT and docetaxel by simultaneously targeting microtubule dynamics, androgen signaling, and protein synthesis pathways. The combination is currently being evaluated in a multicenter prospective clinical study led by Zhongda Hospital to determine if it can enhance pathological response depth and improve long-term patient outcomes.
