The combination of docetaxel and gemcitabine is a chemotherapy regimen frequently utilized in the treatment of various sarcomas, including uterine leiomyosarcoma. Docetaxel is a taxane that acts as a microtubule stabilizer, binding to tubulin to prevent the disassembly of microtubules, which leads to cell cycle arrest and apoptosis. Gemcitabine is a pyrimidine nucleoside analog that inhibits DNA synthesis by inhibiting ribonucleotide reductase and competing with deoxycytidine triphosphate for incorporation into DNA. In a specific pilot safety and toxicity trial (NCT01533207) sponsored by the Albert Einstein College of Medicine, this combination was evaluated as an adjuvant therapy following complete surgical resection of uterine leiomyosarcoma, often administered in sequence or combination with radiation therapies such as intensity-modulated radiation therapy (IMRT) or brachytherapy.
