This is a combination therapy consisting of the chemotherapy agent docetaxel and a luteinizing hormone-releasing hormone (LH-RH) agonist. Docetaxel is a taxane-class antineoplastic that stabilizes microtubules, inhibiting cell division and inducing apoptosis in rapidly dividing cancer cells. LH-RH agonists (such as leuprolide, goserelin, triptorelin) are hormonal agents that initially cause a surge ("flare") in testosterone production followed by downregulation of the pituitary-gonadal axis, leading to long-term suppression of testosterone levels (medical castration). This combination is primarily used for metastatic hormone-sensitive prostate cancer (mHSPC), where it has been shown to improve overall survival compared to hormonal therapy alone. The synergy arises because androgen deprivation drives prostate cancer cells into mitosis during the initial testosterone flare, making them more susceptible to cytotoxic effects from docetaxel[1][2][5].
