Overview
The DOF regimen is a combination chemotherapy treatment consisting of docetaxel, oxaliplatin, 5-fluorouracil (5-FU), and leucovorin (calcium folinate). This regimen is primarily investigated for the treatment of locally advanced or metastatic gastric carcinoma. Docetaxel is a taxane that stabilizes microtubules, preventing their disassembly and thereby inhibiting cell division. Oxaliplatin is a platinum-based antineoplastic agent that forms covalent cross-links with DNA, leading to DNA damage and apoptosis. 5-fluorouracil is a pyrimidine analog that acts as an antimetabolite, primarily inhibiting thymidylate synthase to disrupt DNA synthesis. Leucovorin is administered alongside 5-FU to enhance its binding affinity to thymidylate synthase, thereby increasing its cytotoxic efficacy. The Chinese PLA General Hospital has conducted Phase 2 clinical research evaluating this combination as a neoadjuvant therapy, both alone and in conjunction with the anti-VEGF antibody bevacizumab, to improve surgical resection rates in patients with gastric cancer.
