Overview

Docetaxel + pelareorep is a combination therapy under investigation for the treatment of various cancers, notably metastatic castration-resistant prostate cancer (mCRPC) and other solid tumors. Docetaxel is a taxane-class small molecule chemotherapeutic agent that promotes microtubule assembly and stabilizes them against depolymerization, thereby inhibiting cell division and inducing apoptosis in rapidly dividing cancer cells[1][3]. Pelareorep is an oncolytic virus (a proprietary isolate of reovirus type 3 Dearing strain) that selectively infects and replicates within cancer cells, leading to direct tumor cell lysis. Additionally, pelareorep induces an inflammatory response in the tumor microenvironment, upregulates PD-L1 expression, enhances T-cell infiltration, and can convert immunologically "cold" tumors into "hot" ones to improve immune recognition[6]. The combination has been evaluated in clinical trials for synergistic effects due to their complementary mechanisms—docetaxel’s cytotoxicity with pelareorep’s immunomodulatory oncolytic activity[2].
