Docetaxel + pemetrexed is an investigational combination chemotherapy regimen consisting of two small molecule antineoplastic agents, docetaxel (a taxane) and pemetrexed (an antifolate). Both drugs disrupt cell division but through different mechanisms. Docetaxel stabilizes microtubules, inhibiting their depolymerization and thereby blocking mitosis. Pemetrexed inhibits several folate-dependent enzymes involved in the synthesis of thymidine and purine nucleotides, leading to inhibition of DNA and RNA synthesis. This combination has been studied primarily as a second-line treatment for advanced non-small cell lung cancer (NSCLC), with the aim of improving efficacy over monotherapy regimens. However, clinical trials have shown that while both agents are effective as single agents in NSCLC, their combination did not demonstrate additional benefit due to increased toxicity[1][3][2]. The regimen remains investigational without regulatory approval for this specific doublet.
