Docetaxel + plinabulin is a combination therapy under investigation primarily for the treatment of non-small cell lung cancer (NSCLC), particularly in patients with EGFR wild-type tumors who have received prior treatments. Docetaxel is a well-established microtubule-stabilizing chemotherapeutic agent, while plinabulin is a novel, investigational small molecule that acts as an immunomodulating microtubule-destabilizing agent. Plinabulin binds to the colchicine-binding site on β-tubulin, disrupting tumor vasculature and promoting dendritic cell maturation and T-cell activation via GEF-H1 activation. This combination has demonstrated improved overall survival and reduced incidence of severe neutropenia compared to docetaxel alone in Phase 3 clinical trials.
