Overview

This is a combination chemotherapy regimen consisting of four agents—docetaxel, ramucirumab, gemcitabine, and pemetrexed. Each component has a distinct mechanism of action: - **Docetaxel** is a taxane-class small molecule that inhibits microtubule depolymerization, disrupting mitosis and leading to cell death. - **Ramucirumab** is a fully human monoclonal antibody targeting vascular endothelial growth factor receptor 2 (VEGFR-2), thereby inhibiting angiogenesis required for tumor growth[2][5]. - **Gemcitabine** is an antimetabolite nucleoside analog that inhibits DNA synthesis by incorporation into DNA strands and inhibition of ribonucleotide reductase. - **Pemetrexed** is an antifolate small molecule that inhibits multiple enzymes involved in folate metabolism and DNA synthesis. These drugs are individually approved for various cancers (notably non-small cell lung cancer [NSCLC], gastric cancer, osteosarcoma), but there are no known clinical studies or approvals for the simultaneous use of all four together as one regimen. Combinations such as docetaxel plus ramucirumab[1][3][5], gemcitabine plus docetaxel[6], or docetaxel plus pemetrexed[8] have been studied separately in NSCLC and other malignancies.
