Overview

Docetaxel + SGN-15 is a combination therapy consisting of docetaxel, a taxane chemotherapeutic agent, and SGN-15 (also known as cBR96-doxorubicin immunoconjugate), an antibody-drug conjugate (ADC). SGN-15 is composed of a chimeric BR96 monoclonal antibody that targets the Lewis Y (Le^y) antigen, which is highly expressed in many solid tumors, conjugated to doxorubicin via an acid-labile hydrazone linker[1][4]. The antibody component binds to the Lewis Y antigen, internalizes rapidly, and releases the doxorubicin payload at the low pH present within tumor cells[6]. This combination has been studied primarily in non-small cell lung cancer (NSCLC), breast cancer, colorectal cancer, and hormone-refractory prostate cancer[2][3][4][7]. Preclinical studies demonstrated synergistic activity between SGN-15 and docetaxel based on cell cycle specificity[1]. The sequence of administration appears to be important, with studies showing a five-fold increase in response when SGN-15 is combined with docetaxel compared to docetaxel alone, and an additional 37% improvement when SGN-15 precedes docetaxel[1]. In clinical trials, the combination has shown promising results in terms of survival and tolerability. A randomized phase II study in second and third-line NSCLC showed a median overall survival of 7.3 months for patients on the combination arm versus 5.9 months on the docetaxel-alone arm, with one-year survival rates of 35% and 21%, respectively[3]. The combination was generally well-tolerated, with the most frequent treatment-related adverse events including nausea, vomiting, anorexia, and fatigue[3][4].
