This drug is a fixed-dose combination of two small molecule agents, donepezil and idalopirdine, developed for the treatment of mild to moderate Alzheimer's disease. Donepezil is a selective, reversible acetylcholinesterase inhibitor that increases acetylcholine levels in the brain, thereby enhancing cholinergic neurotransmission and improving cognitive symptoms in Alzheimer's disease. Idalopirdine is a selective 5-hydroxytryptamine 6 (5-HT6) receptor antagonist; it modulates neurotransmitter systems involved in cognition by blocking 5-HT6 receptors expressed on glutamatergic neurons and GABAergic interneurons. The rationale for combining these drugs was to potentiate the effects of acetylcholinesterase inhibition with additional modulation of serotonergic pathways to further improve cognitive function. Clinical trials showed that while this combination was generally safe and well tolerated, it did not demonstrate significant efficacy over standard therapy[1][2][3][5].
