Overview

Isolated cadaveric islet cells, generically known as **donislecel**, are allogeneic pancreatic islets of Langerhans harvested from the pancreata of deceased human donors. This cell therapy is indicated for the treatment of Type 1 Diabetes in adults who are unable to approach target glycated hemoglobin (HbA1c) levels due to repeated episodes of severe hypoglycemia despite intensive diabetes management. The islets are purified and typically infused into the recipient's hepatic portal vein. Once engrafted in the liver, the islet cells—which include insulin-producing beta cells—function as a biological insulin delivery system, secreting insulin in response to physiological blood glucose fluctuations. This approach aims to restore glycemic control, reduce or eliminate the need for exogenous insulin, and prevent life-threatening hypoglycemic unawareness. The procedure is often performed using the Edmonton Protocol, which employs a steroid-free immunosuppressive regimen to prevent graft rejection.
