Donor-derived ex-vivo expanded natural killer (NK) cells are an adoptive cellular immunotherapy product. These cells are isolated from a healthy donor, typically a human leukocyte antigen (HLA)-haploidentical relative, and expanded ex-vivo using cytokines (such as IL-2 or IL-15) to reach a high therapeutic dose. In the context of the EXCEL study (NCT04347616), these cells are administered to pediatric and young adult patients with high-risk acute myeloid leukemia (AML) who are undergoing haploidentical hematopoietic cell transplantation (haploHCT). The therapeutic goal is to enhance immune reconstitution and provide a graft-versus-leukemia (GVL) effect. NK cells are particularly effective in the haploidentical setting due to KIR-ligand mismatch, which triggers NK cell activation against residual leukemia cells without significantly increasing the risk of graft-versus-host disease (GVHD).
