This therapeutic regimen, developed at Stanford University, is an investigational protocol designed to induce immune tolerance in adult HLA-matched living donor kidney transplant recipients. The procedure utilizes a non-myeloablative conditioning phase consisting of total lymphoid irradiation (TLI) and anti-thymocyte globulin (ATG), followed by the infusion of donor-derived hematopoietic stem cells (CD34+ cells) and T cells (CD3+ cells). By establishing a state of transient or persistent mixed chimerism, the regimen aims to "re-educate" the recipient's immune system to recognize the donor organ as self. The primary clinical objective is to facilitate the complete and safe withdrawal of maintenance immunosuppressive drugs, thereby avoiding long-term medication toxicities while preventing graft rejection.
