Overview

Donor hematopoietic stem and progenitor cell (HSPC) infusion is an experimental cell therapy developed by the University of California, Los Angeles (UCLA) to induce immune tolerance in liver transplant recipients. The therapy involves the infusion of a donor-derived, CD34-enriched cell product containing a specific dose of CD3+ T-cells into a recipient who has previously received an HLA-matched living-donor liver transplant. This infusion follows a conditioning regimen of total lymphoid irradiation (TLI) and rabbit anti-thymocyte globulin (rATG). The primary mechanism is the induction of durable mixed chimerism, which "retrains" the recipient's immune system to recognize the donor organ as self, potentially allowing for the complete withdrawal of maintenance immunosuppressive drugs and avoiding their long-term toxicities.
